By filling out this form every time you complete a written assignment, you will be able to keep track of your progress and performance. To earn a passing grade, you must complete ALL ASSIGNMENTS*. If an assignment is missing, you have until the portfolio due date to complete it. Please use this form as an electronic document and always fill it out on the computer. Check the appropriate box next to an assignment by using the plus sign (+). Don’t forget to save your file and If you complete an assignment late, please indicate how many days you were late. You need to submit this form with your Portfolio Cover Letter. You will also post it in your Personal Folder.
